Requesting and claiming care
In our step-by-step guide, you’ll see that you can easily manage and claim many types of care yourself via Mijn Aevitae. For some care requests, a claim form is required. You can send this to vitaaldeclaraties@aevitae.com, after which the healthcare provider will contact you.

You can find your policy in Mijn Aevitae, under the heading 'Mijn Zorgverzekering'.
You received your username in the welcome letter at the beginning of December 2024. You can set a password yourself when creating your account.
You can log in to Mijn Aevitae via iDIN. You then log in with your bank details and don't need the welcome letter. More information about activating or logging in with iDIN? Then click here or visit www.idin.nl.
Would you rather log in with your username? Then you can request a copy of your welcome letter via our contactform.
If you want to make use of the reimbursement for physiotherapy and you go to a contracted physiotherapist, the physiotherapist can send the claim directly to Aevitae. You do not need to request anything in advance, you can contact the physiotherapist directly. Check our Care Finder to see which physiotherapists are contracted in your area.
Do you want to take advantage of the reimbursement for the budget coach or dietician? Then complete the MyDailyLifestyle application form and send it by email to vitaldeclaraties@aevitae.com.
For all other reimbursements of the IKEA Inzetbaar module, we refer to the healthcare providers mentioned in the policy conditions. You will find the policy conditions in Mijn Aevitae.
There is a waiting period for the reimbursement of orthodontics, unless you already had a package with the previous health insurer with a comparable reimbursement for orthodontics, with a minimum term of one year prior to the start date of the policy with Aevitae. If that is the case, the waiting time at Aevitae will not apply.
We prioritize your privacy. No personal care consumption data is shared with IKEA by Aevitae. IKEA will not share more data than absolutely necessary to offer you the IKEA Inzetbaar Module free of charge. For any questions, you can consult the privacy statement on Aevitae’s website or ask a colleague from P&C.
